Frequently Asked Questions


1.     What do I need to do before calling for an appointment?

·         We accept most major insurance.  If you are unsure about your coverage, give our office a call at 302-827-4365.

·         When you call for an appointment, please have your insurance card and policy number ready.

 

2.     What should I bring to my appointment?

·         Current insurance card

·         Driver’s license or other acceptable photo I.D.

·         Medicare card if applicable

·         Any medications that you are currently taking.  Actuals are preferred, OR an accurate list of medications, including dosages and how often you take them.  Please include all vitamins and over the counter medications.

·         New patients and patients with changed insurance information should arrive 15 minutes prior to your appointment time to allow for the completion of new and updated patient information. New patients can save time by completing the forms in the Document Library (below) in advance.

 

3.     What if I need to cancel an appointment?

·         Please provide at least 24 hours’ notice if you must cancel your appointment.  We will charge a $45 fee if you cancel an appointment less than 24 hours in advance.  The fee must be paid prior to your next visit.  Also see our Office Policies and Service Agreement.

 

4.     What is the billing procedure?

·         We are required to collect your co-pay and any co-insurance, as applicable, at the time of service.  We accept cash, check and major credit cards.

·         If we do not participate in your insurance plan, you are required to pay for the service at the time of the visit.

 

 

5.     How do I obtain prescriptions or refills?

·         We can send prescriptions electronically to the pharmacy of your choice, or to your mail order pharmacy.

·         When you need a refill, just call our office and provide the following information.  We will complete the process for you.

o    Name and dosage of the drug

o    30 day or 90 day refill

o    Name and address of pharmacy

 

6.     What are your office hours?

 

              Monday and Tuesday 8:00am to 4:30pm

              Wednesday - CLOSED

                     Thursday and Friday 8:00am to 4:30pm
